S.T.A.R.
WITH TEENS RELEASE KNOW ALL PERSONS BY THESE PRESENTS, that,

(hereinafter referred to as “Participant”), desires to participate in the
activities with S.T.A.R. with Teens (hereinafter referred to as “S.T.A.R.”
which is intended to include its agents, employees, representatives
successors, assigns and all those in privity with
them) at the program project which will be held at,
________________________________________________________(site location). FOR AND IN CONSIDERATION OF THE ABOVE
OPPORTUNITY, Participant does hereby forever and finally release, remise,
acquit, satisfy and forever discharge S.T.A.R. of and from all manner of
action and actions, cause and causes of action, suits, debts, dues, sums of
money, bonds, billing, contracts, controversies, agreements, promises,
damages, variances, judgments, executions, claims and demands whatsoever, in
law or equity, which may arise or might in the future arise or hereinafter
may arise for or against S.T.A.R., its agents, employees, representatives,
successors assigns and all those in privity with
them, for all present and future damages resulting from all incidents no
matter how caused even those resulting from the negligent acts or omissions
of S.T.A.R. This document is meant to a full and complete
release from any and all liabilities that may arise from Participant’s
activities with S.T.A.R. even though said liability or liabilities are
predicated or based upon the alleged negligent acts of omissions of S.T.A.R.
This release is given freely and voluntarily by the Participant and their
legal guardian and is meant to remain in existence throughout the duration of
